Alterations / Amendments to the Passport
A Sri Lankan passport holder may apply for amendments of the data included in his/her passport and if the request is accepted by the Department, all such amendments will be included in the observation and alterations page. (Download Application Form here)
Required Documents
- Duly completed application form (Download Form ‘O’). Fill the application using English Block Letters
- Original Sri Lankan Passport
- Documentary evidence to prove the accuracy of the alteration or amendment
- Note: Originals with photocopies should be submitted

5. Valid visa of the current country of residence
