ELIGIBILITY
A death can be registered under the Section 7 of the Consular Functions Act if the deceased is a citizen of Sri Lanka and the death had occurred in Oman or concurrently accredited country; Yeman.
The declaration should be submitted within three (3) months of such death to the relevant Diplomatic/Consular officer of the Embassy. If not registered, after three (3) months and within one year, the Diplomatic/Consular officer should record the reason for the delay and this will affect the registration. After one year, if the registration has not been done, the consent of the Registrar General Department is required to register the death.
The declaration can be made by any one of the following persons;
- The nearest relative of the deceased who was present at the time of death or in attendance during the last illness of the deceased
- A relative of the deceased who resides in the country where the death occurred
- If there is no such relative, any person who was present at the death
- The main occupant of the house where the death occurred
- The doctor in charge of the hospital or public institution where the death occurred
- If there are no such persons, the person who takes action to dispose of the body of the deceased by burial, cremation or by any other means
Required Documents
- Declaration form filled by the declarant along with the documents to prove the identification of the declarant. (Download Form here)
- Valid Sri Lankan Passport of the deceased or any other available identification documents of the deceased to prove Sri Lankan citizenship
- Death certificate of the deceased issued by the Competent Authority of the country / Hospital in which the death took place along with the English translation of the Death Certificate attested by the Foreign Ministry of the issued country must be attached if the document issued in a language other than Sinhala, Tamil or English)
- If the cause of the death is not included in Death certificate issued by the relevant country, it should be proved by supporting documents.
- Original Birth certificate of the deceased
- Original Death certificate of the deceased
- Cargo receipts will be needed, if the death is been registered in Sri Lanka
- Resident Permit of the applying Person (Document issued by the Immigration Authority of the Country of Residence indicating the status and category of your visa
SUBMISSION PROCEDURE
If Declarant is living in Oman or an Accredited country and death occur in Oman, the declaration for registration of a death should be forwarded to the Embassy of Sri Lanka in Muscat, Oman
Applicants are kindly requested to get an online appointment prior to visiting the Embassy by accessing the Embassy website.
Register the Death at Central Record Room
If it is not been registered and if the declarant is living in Sri Lanka, the declaration for registration of death should be forwarded to the Central Record Room of the Registrar General’s Department, Maligawatte, Colombo 10.
Note: Prior to visiting the Central Record Room, applicants are advised to visit the Consular Affairs Division of the Ministry of Foreign Affairs of Sri Lanka located at 2nd Floor of the Ceylinco Building, Colombo 01, to pay the fee. Central record room and Consular Division are open from 9.00 a.m. to 3.00 p.m. except for Saturdays, Sundays and public holidays.
You may submit the application to the Consular section of the Embassy by post or by hand.
The Documents are accepted by the Embassy Consular section between 8.00 a.m. and 2.00 p.m. from Sunday to Thursday.

Note:
- Submission of forged documents and making false declarations is a punishable offense.
- The Embassy is not responsible for any document lost in the post.
